For Wholesale Registration, you just need to follow these steps:

Add a new Page for Wholesaler Registration Form
Place the shortcode “[wwp_registration_form]” into that page
Form Fields:Email,Username and Password.
Go to WooCommerce Settings select “Wholesale Pricing” tab, there is an option to select registration page, select the registration page you just created now.
Save your Settings.
