It's that simple, fill out the form, and click Send.
Please note that you CANNOT receive emails to your spoofed address, and if you send a spoofed email to GMAIL from GMAIL it will give them a warningtelling them that this may not have come from an actual gmail service. So it's all ways better to use @FBI.gov, or @hotmail.com or whatever you wish to use.
